  1. My department/division has Mission and Values statements that were developed in collaboration with my staff using inputs solicited from all departmental/divisional employees. Several values relate specifically to our treatment of customers and employees.
  2. The Mission and Values statements reflect what my staff and I stand for and we use them as the foundation for our day-to-day decision-making.
  3. The Mission and Values statements are owned by a majority of department/division employees as reflected in their connection with each other and the nature of their responses to serve our customers.
  4. We have identified our internal and external customers, understand their requirements, and engage in a routine dialogue to stay current on any changes in those requirements.
  5. We communicate proactively with all sister departments/divisions and vendors that serve us to explain our requirements and deliver feedback whenever requirements are not met or service is extraordinary.
  6. We have a department/division business plan (separate from the budget) that highlights mission critical initiatives and the specific accomplishments that support those initiatives. Everyone in the department/division knows and understands the connection between their job and the initiatives
  7. We allocate resources based on the business plan and reallocate them if business conditions change. Most employees understand and support these decisions without griping.
  8. The critical work processes in our department/division are documented, have assigned process owners, and are implemented with discipline.
  9. My direct staff and I know the key process measurements that determine departmental success. We monitor these measurements routinely.
  10. The designated process owners understand these measurements, monitor them, and can make the appropriate interventions in response to deviations from standard.
  11. When process measurements in any area are chronically below expectations, the designated process owners analyze the cause(s) of the undesirable performance and make process improvements.
  12. I face problems and setbacks with realism and pragmatism, responding appropriately to what is really confronting me. I seldom overreact out of an inflated perception of threat or fail to act out of a wishful sense that the cause of the circumstance will correct itself.
  13. Our department/division is truly customer focused (both internal & external). Employees enjoy being of service and feel supported by me and my staff in making the in-the-moment decisions required to resolve customer issues in a manner that delights them.
  14. I have a good relationship with my boss and feel comfortable expressing myself fully to him/her about any issue.
  15. My boss uses a negotiated leadership style to support me so that I get both the direction and autonomy I need based on my confidence and competence in each area where I am responsible for producing results.

  17. I spend much of my time as a coach and mentor helping my direct staff and selected others to be successful and insuring that business results are achieved in ways that are aligned with the values we assert.
  18. My department/division has a reputation for getting the job done will and developing people in the process. We are a recognized source of talent when openings occur in other departments/divisions and always have an abundance of applicants for our own openings.
  19. Morale is very high in my department/division. We reward and recognize people in financial and non-financial ways, meaningful to them, that demonstrate our authentic appreciation of their contributions.
  20. We understand the personal qualities and skills that are required for success in every job in our department/division and know how to find and attract people who possess those qualities and skills.
  21. I have a staff of direct reports who were consciously chosen because they complemented me i.e. they have strengths in areas where I know I am weak.
  22. I feel comfortable that I can go away on vacation for a month with no staff contact and have my department/division operate smoothly and within budget.
  23. I live a balanced life with ample time for my family, friends, and my personal interests.
